2) Click “Create New Case” then click “Case Entry”

3) Fill out the Digital Lab Script.

4) Click “Attach File” and upload x-rays scans, photos, or any other file.

2) Call 716.839.8008 for a pick up

Pick up requests after 3:30 pm will be picked up the next business day. If your practice is within a 60 mile radius of the Buffalo-Niagara Intl. Airport, we offer FREE daily pickup and delivery by our polite drivers.

For clients practicing outside our regional service area we provide shipping labels for your convenience.

2) Call 716.839.8008 for a shipping label
(Shipping label will be emailed or faxed)

3) Attach shipping label to box.

Please allow 1-3 days, depending on location, for your case to arrive at Evolution Dental Science. Rush cases are available.
